I, the undersigned, hereby confirm my enrollment in the Hamdan EFQM Certified Assessor Program, delivered by the Hamdan Bin Rashid Al Maktoum Foundation for Medical and Educational Sciences, in collaboration with EFQM, and sponsored and funded by the Hamdan Bin Rashid Al Maktoum Foundation for Medical and Educational Sciences.

I acknowledge that I have reviewed all details and requirements related to the program, and I undertake to commit to the following:

1. Attend and participate consistently in all program activities in accordance with the schedule provided by the Foundation, as a condition for receiving the "Certified Assessor" certificate.
2. Complete all program requirements and assigned tasks as detailed in the program documentation and attachments.
3. Participate in assessment cycles related to the Hamdan EFQM Educational Model as per the instructions and timelines set by the Foundation.
4. Maintain the confidentiality of all data and information received from the Foundation, especially those that are by nature confidential, not publicly available, and/or labeled as confidential. It is strictly prohibited to use, disclose, or publish such information without obtaining prior written approval from the Foundation.
5. Acknowledge that all intellectual and literary property rights related to the program are the sole property of the Foundation, and I commit not to use, reproduce, or share any part of the program in any form.
6. Appear in a respectful and professional manner, and adhere to appropriate dress codes during participation in all program activities.
7. Provide support to schools participating in the Hamdan EFQM Educational Model assessments and commit to actively participate in at least THREE assessment cycles.

In the event of breaching any of the above commitments, the Hamdan Bin Rashid Al Maktoum Foundation for Medical and Educational Sciences reserves the full right to withdraw the grant, and to request reimbursement of all expenses paid for my participation in the program, amounting to AED 10,000, in addition to taking any necessary legal action.
